Testimonial Release Agreement

I hereby represent that I am at least 18 years of age, that I have the right to contract in my own name and that I agree to allow Mediatrition, Inc., Know the Cause® and its designees to use, publish and disclose the information I provide herein, including but not limited to my name, biographical information, and my story (my “Testimonial”), for marketing, advertising and public relations in all media, including but not limited to internet, websites, television, and social media.

In exchange for good and valuable consideration, including the promises contained herein, the receipt and sufficiency of which is hereby acknowledged, I, the undersigned, hereby grant to Mediatrition, Inc., Know the Cause® and its designees the right to use my Testimonial, in whole or in part, and without restriction as to changes or alterations, for marketing, advertising and public relations purposes in all media, including but not limited to internet, websites, television, and social media.

I understand and approve the disclosure of my Testimonial to the media and other individuals and entities that may be involved in the marketing, advertising and public relations efforts of Mediatrition, Inc., Know the Cause® or its designees. I understand and acknowledge that the media may be interested in telling my story, and I am willing to cooperate and participate in media interviews as they arise. Mediatrition hereby agrees to provide me with assistance with respect to any such interviews. The rights granted herein shall extend in perpetuity, throughout the world and for any purpose whatsoever, including but not limited to marketing, advertising and public relations, and in all media, including but not limited to internet, websites, television, and social media.

I acknowledge that Mediatrition, Inc. and Know the Cause® have no obligation to return any photographs or images to me.

I hereby RELEASE, WAIVE AND FOREVER DISCHARGE Mediatrition, Inc, Know the Cause® and/or its designees, from any and all claims arising out of, or in connection with, the use by, including but not limited to any and all claims for damages of any kind based on the use of my Testimonial or the information contained therein.

I understand that this is the entire agreement between me and Mediatrition, Inc., Know the Cause® or its designees, as to the subject matter contained herein.
